As the Quality Manager at Emmanuel Hospice, you will experience a rewarding work life in a full time, salaried position.
Responsibilities
- Work a hybrid schedule of some office work and some work from home. Serves on the Operational Leadership Team and on the Manager on Call rotation with rotating on-call responsibilities.
- Oversees, supports and coordinates the Organization-wide quality program in a manner in keeping with the mission of Emmanuel Hospice and in compliance with license and regulatory standards.
- Assists in all efforts for survey preparation before and during survey and assists Organization in meeting all post survey requirements.
- Coordinates the review, development, and communication of non-Human Resources related policies and procedures and recommends the need for changes in Organizational policies and programs as needed.
- Develops and implements Organization-wide processes for improving Organizational performance, identifies improvement opportunities, develops improvement plans, and utilizes resources effectively to coordinate the Quality Assurance Process Improvement Program.
- Collaborates with appropriate team members to develop, implement, and sustain specialty programming to enhance patient care.
- Develops quality metric reporting tools for the Quality Committee of the Board of Directors, leads Quality Committee meetings, solicits feedback from the committee, and implements new processes as appropriate.
- Collaborates with appropriate team members to provide education to clinical staff on relevant clinical, regulatory, quality related topics, and identified Organizational improvement opportunities.
Qualifications
- Holds and maintains a certification and/or licensure to be a member of a hospice clinical team.
- Bachelors or Master’s Degree preferred
- 5 years of healthcare experience required, with a minimum of three year’s related work experience in quality management. Leadership experience preferred.
- A thorough understanding of theory and practices of performance improvement in a hospice or home health environment

As a Per Diem Hospice Social Worker at Emmanuel Hospice, you will experience a rewarding work life while contributing to our team in a per diem, hourly position.
Responsibilities
- Working a variable Per Diem schedule (minimum requirements: 2 shifts per month, 4 weekend shifts per year, and 1 holiday per year plus additional meetings as required)
- Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI.
- Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.
- Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventions.
- Working in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of medical care.
Qualifications
- Current LMSW or LBSW degree required
- Hospice, home care, case management or acute care experience preferred.
- Skilled in physical, emotional and spiritual symptom management while working in conjunction with an IDT and functioning independently in the field.
- Flexible in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities.

As a Mid-Shift Hospice Aide (CNA) at Emmanuel Hospice, you will experience a rewarding work life while contributing to our team in a full time, hourly position.
Responsibilities
- Working a schedule of Monday - Friday 1-9pm with a rotation of weekends and holidays
- Providing daily cares and comfort to the dying and their loved ones in homes and facilities within a 50-mile radius around Grand Rapids, MI.
- Providing personal hands-on care and activities of daily living under the direction of a professional nurse.
- Self-scheduling visits based on patient and family needs.
- Working with a dynamic CNA team to ensure patient care is delivered consistently and excellently.
Qualifications
- Certified as a Certified Nurse Aide (CNA) in the State of Michigan.
- Must have a reliable vehicle to travel to patient visits.
- Previous experience as a CNA required, Hospice or Home Care experience preferred.

Responsibilities
- Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI.
- Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.
- Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventions.
- Working with community Physicians and the Hospice Medical Director to ensure that each patient has individualized medical care including medications, equipment, and supplies that work towards meeting patient and family goals.
- Regularly administering care to a lower-than-average caseload.
- Working creatively in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of medical care.
- Educating patients, families, caregivers, and community members about hospice services, and issues related to end of life.
- Providing exemplary care and service to all those you encounter in your work.
Qualifications
- Current RN degree required; hospice, home care, case management or acute care experience required.
- The successful candidate will be skilled in physical, emotional and spiritual symptom management while working in conjunction with an IDT and functioning independently in the field.

Are You Called to Serve? Finding the right hospice is about finding the right people. People who will delicately guide patients through an unknown journey ahead; who serve the needs of family members with the same care as their patients; who will always be there to answer questions, so no one is left in the dark. As an Extended Care Registered Nurse (RN) at Emmanuel Hospice, you will serve alongside a dynamic team in a full-time (salaried) position.
Responsibilities
- Schedule is 7 days on/7 days off with a combination of nights and weekends. A holiday rotation and other occasional staff meetings also required.
- Triaging after hours needs of patients and families through both phone contact and visits on a rotating schedule of evenings/nights and on the weekends alongside a team of Registered Nurses (RN)s
- Providing after hours crisis management for terminally ill patients and their families on an on-call basis during evenings and weekends
- Conducting informational visits regarding hospice services and nursing admission assessments on a rotating schedule of evenings and weekends as required
- Communicating effectively with other after hours hospice RNs and other staff to ensure coordinated care is provided.
Qualifications
- Current registered nurse (RN) license required
- Hospice, home care, emergency room or ICU experience preferred
