Responsibilities:
- Member of the Clinical Operations Team; responsible for implementing strategy, cascading information appropriately within the team, and ensuring a healthy culture and team exists.
- Oversees day-to-day care management by leading the Patient Care Coordination Team
- Directs and oversees patient care while ensuring that Quality Outcomes, metrics, and KPIs are met according to industry standards and organizational expectations.
- Serve as the liaison between the Clinical Team/Supports Coordinators and Senior Leadership, ensuring alignment on organizational goals, patient care standards, and resource management.
- Regularly review the performance of clinical staff, working closely with the coordinators to identify areas for improvement and support training initiatives..
- Provides budget and resource management adhering to organizational financial goals and metrics. Ensuring high quality patient care while managing costs.
- Responsible for ensuring the highest level of customer service is delivered to patients, families, facilities and vendors by promoting collaborative care that leads to quality patient outcomes.
- Oversees leaders who directly oversee clinical teams, including hiring, evaluating, and providing corrective action of team members with the assistance of the Vice President of Patient Care and Compliance and Human Resources as needed.
Qualifications:
- Valid Michigan license as Registered Nurse, Licensed Master of Social Work, or experience in leading healthcare teams of greater than 30 people required.
- Bachelors degree required, Masters Degree preferred.
- Progressive leadership experience required.
- 5 years of healthcare required, hospice and/or acute care unit experience preferred.
- Experience in Electronic Medical Records and Microsoft Office products is required.

Responsibilities
- Working Monday-Friday including a weekend and holiday rotation
- Full Time: 40 hours, 11am-7pm, Part Time: 16-24 hours, 4pm-8pm.
- Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI

Responsibilities
- Working 12 hour shifts on the weekend (part time or full time shifts available)
- Part Time = every Saturday and Sunday
- Full Time = every Saturday and Sunday + either Mondays or Fridays
- Assisting the after hours team and additional meetings and holidays as required for the role.
- Providing holistic care to the dying and their loved ones in a 50 mile radius of Grand Rapids, MI.
- Assessing the physical, psychological, social and spiritual needs of terminally-ill patients alongside our weekend/extended care team
- Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventions.

As an Extended Care Registered Nurse (RN) at Emmanuel Hospice, you will serve alongside a dynamic team in a full-time (salaried) position.
Responsibilities
- Schedule is 7 days on/7 days off with a combination of nights and weekends. A holiday rotation and other occasional staff meetings also required.
- Triaging after hours needs of patients and families through both phone contact and visits on a rotating schedule of evenings/nights and on the weekends alongside a team of Registered Nurses (RN)s
- Providing after hours crisis management for terminally ill patients and their families on an on-call basis during evenings and weekends
- Conducting informational visits regarding hospice services and nursing admission assessments on a rotating schedule of evenings and weekends as required
- Communicating effectively with other after hours hospice RNs and other staff to ensure coordinated care is provided.
