  • Clinical Manager

    Responsibilities

    • Maintaining and building a team of 30+ qualified Nurses, Social Workers, Spiritual Caregivers, and Hospice Aides who provide patient-centered care in coordination with an Interdisciplinary Team.
    • Supporting, coordinating and overseeing the day to day care of patients and families in a manner in keeping with the mission of Emmanuel Hospice and in compliance with hospice conditions of participation, state regulations, CHAP standards and organizational policies and procedures.
    • Member of a dynamic Operations Leadership Team assisting with day to day leadership and clinical operations.
    • Overall management and oversight of exemplary care and compliance of the patient care services provided by Emmanuel Hospice in coordination with the Medical Director.
    • Directing the work-flow of field teams to ensure that coordinated and exceptional end-of-life care is delivered to those we serve.
    • Assisting in ensuring compliance with all applicable state, federal regulations, CHAP standards and organizational policies and procedures.
    • Developing a world-class Hospice Interdisciplinary Team in a manner consistent with the mission, vision and values of the organization.
    • A rotation in the manager on call schedule is required which includes some evenings, weekends and holidays

     
    Qualifications:

    • Current license as an RN or MSW in the State of Michigan or experience leading healthcare teams of greater than 30 people.
    • Minimum of 5 years of healthcare experience required.
    • Exemplify organizational and clinical while multi-tasking & maintaining focus in a fast-paced, rapidly growing environment.
    • Flexibility in adapting to the changing needs of the industry, organization, as well as our patients and families.
    • Proficient in Microsoft programs for internal and external communications, database tracking/reporting, and spreadsheet applications.
    • 2 year of hospice experience preferred.
    • Masters degree preferred.

  • Hospice Aide

    Responsibilities

    • Providing holistic care to the dying and their loved ones in a versatile service area in a 50-mile radius around Grand Rapids, MI.
    • Under the professional nurse’s direction, the Hospice Aide is responsible for providing personal hands-on care personal care and activities of daily living.
    • Providing daily cares and comfort for the patient.
    • Working in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of spiritual, psychosocial, and physical care.

     
    Qualifications

    • Certified as a Certified Nurse Aide in the State of Michigan
    • This position requires flexibility in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities.
    • Previous experience as a CNA required, Hospice or Home Care experience preferred.
    • The Hospice Aide should be able to respond sensitively to patient/family needs by observing and reporting any changes, including physical, spiritual, social and emotional, to a registered nurse or other members of the Interdisciplinary Team.
    • The Hospice Aide reports to the RN Case Manager and overall care is overseen by the Director of Clinical Services.

  • RN Case Manager

    As an RN Case Manager, you will experience a rewarding work life in a full time, salaried position.

    Responsibilities

    • Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI.
    • Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.
    • Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventions.
    • Working with community Physicians and the Hospice Medical Director to ensure that each patient has individualized medical care including medications, equipment, and supplies that work towards meeting patient and family goals.
    • Regularly administering care to a lower-than-average caseload.
    • Working creatively in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of medical care.
    • Educating patients, families, caregivers, and community members about hospice services, and issues related to end of life.
    • Providing exemplary care and service to all those you encounter in your work.

     
    Qualifications

    •  Current RN degree required; hospice, home care, case management or acute care experience required.
    • The successful candidate will be skilled in physical, emotional and spiritual symptom management while working in conjunction with an IDT and functioning independently in the field.
    • This position requires flexibility in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities.

  • Per Diem RN

    Responsibilities

    • Assists with providing coverage and support for RN Case Managers, Extended Care Nurses, and Admissions Nurses during periods of increased need and to cover time off for full-time staff.
    • Providing Primary and On-Call Services triaging needs of patients and families through both phone contact and visits in the evenings and on the weekend.
    • Providing routine and crisis management for terminally ill patients and their families.
    • Conducting informational visits regarding hospice services.
    • Completing admission assessments as requested.
    • Communicating effectively with other hospice staff to ensure coordinated care is provided.
    • Shifts will vary based on the needs of the organization. Must be willing to work all of the varied shifts/roles in which care is provided by Emmanuel Hospice nursing staff.

     
    Qualifications

    • The successful candidate will be a highly skilled and flexible clinician with excellent triage skills of individual’s physical, emotional and spiritual symptoms.
    • This position requires the ability to be a calming presence in crisis situations and effectively manage multiple calls at the same time.
    • This position requires flexibility in adapting to the changing needs of the patient and family.
    • Current RN degree and acute care experience required; hospice, home care, emergency room or ICU experience preferred.
