As a Weekend Admissions RN at Emmanuel Hospice, you will serve alongside a dynamic team in a full time, salaried position.

Responsibilities

  • Working a schedule of every Saturday, Sunday, and Monday 9am - 9pm and additional meetings and holidays as required for the role.
  • Serving alongside our extended care team to to provide nursing services to the dying and their loved ones in a 50 mile radius of Grand Rapids, MI.
  • Conducting informational and sign-on visits
  • Completing comprehensive RN admission assessments
  • Collaborating with referral sources, patients, families, and healthcare professionals to establish an initial plan of care
  • Communicating effectively with other hospice registered nurses and other staff to ensure coordinated care is provided
  • Assists with follow-up contacts and re-assessments as needed

Qualifications

  • Current registered nurse (RN) license required
  • Hospice, home care, case management or acute care experience required
  • Experience with EMRs required

As a Bereavement Coordinator at Emmanuel Hospice, you will experience a rewarding work life while contributing to our team in a full time, salaried, exempt position.

Responsibilities:

  • Works in a hybrid model of office and some work from home.
  • Provides ongoing assessments of bereavement needs of the patient/family/patient caregiver and initiates appropriate intervention or referral.
  • Provides and submits in a timely manner, accurate and appropriate documentation of patient/family services as defined in Emmanuel Hospice policies.
  • Plans, directs and coordinates all bereavement services for all bereaved for at least thirteen (13) months following death of the patient.
  • Special activities of remembrance.
  • Coordinates with community organizations to ensure a variety of bereavement services and programming is available in the community, including educational in-services/seminars and support groups/1:1 counseling as necessary related to grief and loss.
  • Provides referrals to outside agencies or consultants when appropriate.
  • Assists in providing education to Emmanuel Hospice team members regarding grief and loss and how it relates to aspects of Patient/Family Care.

 

Qualifications/ Experience:

  • A degree in Social Work, Counseling, or related field preferred with sufficient training and education to provide bereavement services.
  • 2 years of experience in Counseling/Social Work with hospice experience preferred.
  • Experience in electronic databases and Microsoft Office products required.

As an Extended Care RN at Emmanuel Hospice, you will serve alongside a dynamic team in a full-time (salaried) position.

Responsibilities

  • Schedule is 7 days on/7 days off: 4:30pm-8:30am Monday – Friday and 4:30pm Friday - 8:30am Monday, plus a holiday rotation and other required staff meetings
  • Triaging needs of patients and families through both phone contact and visits in the evenings and on the weekends alongside a team of RNs
  • Providing crisis management for terminally ill patients and their families
  • Conducting informational visits regarding hospice services
  • Completing admission assessments as requested
  • Communicating effectively with other hospice staff to ensure coordinated care is provided

Qualifications

  • Highly skilled clinician with excellent triage skills of individual’s physical, emotional and spiritual symptoms
  • Ability to be a calming presence in crisis situations and effectively manage multiple calls at the same time
  • Flexibility in adapting to the changing needs of the patient and family
  • Current RN degree and acute care experience required
  • Hospice, home care, emergency room or ICU experience preferred

  • Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.

  • Working in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of medical care.

Qualifications

As the Education Manager at Emmanuel Hospice, you will experience a rewarding work life in a full time, salaried position.

Responsibilities

  • Work a hybrid schedule of some office work and some work from home. Serves on the Operational Leadership Team and on the Manager on Call rotation with rotating on-call responsibilities.
  • Oversees, supports and coordinates the Organization-wide education program in a manner in keeping with the mission of Emmanuel Hospice and in compliance with license and regulatory standards.
  • Provides oversight and direction of the Infection Control program and oversees the Infection Prevention/Education Coordinator.
  • Coordinates general and ongoing orientation planning and implementation, including appropriate utilization of other staff resources, scheduling dates and times with new hires, and delivering education.
  • Coordinates initial and annual competency evaluations and annual education programming in compliance with license and regulatory standards and assessed Organizational needs.
  • Ensures that all clinical staff maintain appropriate licensing and complete licensing requirements in a timely manner in compliance with state licensing rules in collaboration with Human Resources.
  • Develops and delivers education to a variety of community audiences and other healthcare professionals in collaboration with the Business Development department as appropriate. This includes the development and delivery of content that allows healthcare and other professionals to obtain CEUs.
  • Collaborates with appropriate team members to provide education to clinical staff on relevant clinical, regulatory, quality related topics, and identified Organizational improvement opportunities.

Qualifications

  • Valid Registered Nurse license in the State of Michigan.
  • Bachelor’s or master’s degree preferred. Associate degree required.
  • 5 years of healthcare experience required, 2 years of acute care and/or hospice experience preferred.
  • Leadership team experience with education delivery, quality assurance, and/or policy development preferred.

Responsibilities

  • Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI.

  • Preferred hours of Monday - Friday 1pm-9pm, hours can be flexible based on candidate's schedule
  • Providing daily cares and comfort to the dying and their loved ones wherever they call home in territories within a 50-mile radius around Grand Rapids, MI.
  • The Hospice Aide is responsible for providing personal hands-on care and activities of daily living under the direction of a professional nurse.
  • Self-scheduling visits based on patient and family needs.
  • Working in unison with an Interdisciplinary Team, including the patient and their support system, to observe and report patients' spiritual, psychosocial, and physical needs.

Qualifications

  • Certified as a Certified Nurse Aide (CNA) in the State of Michigan
  • Must have a reliable vehicle to travel to patient visits.
  • This position requires flexibility in adapting to the changing needs of the patient and family with weekends (every 7th weekend), and holiday (1 per year) on-call responsibilities.
  • Previous experience as a CNA required, Hospice or Home Care experience preferred.
  • The Hospice Aide should be able to respond sensitively to patient/family needs by observing and reporting any changes to a registered nurse or other members of the Team.
  • The Hospice Aide reports to the RN Case Manager and overall care is overseen by the Director of Clinical Services.
