As an Executive Assistant at Emmanuel Hospice, you will experience a rewarding work life while immersed in a full time, hourly position..

Responsibilities

  • Assists the Executive Director with project management
  • Enters data into DonorPerfect software and generates reports
  • Assists with generating, mail merging, and sending letters to donors and families
  • Makes travel arrangements for staff as needed
  • Uses great attention to detail to complete activities in a timely and compliant manner
  • Takes dictation and minutes and accurately enters data
  • Produces reports, presentations and briefs
  • Acts as a point of contact between leadership and internal/external clients
  • Provides support to Fundraising, Administration, and Medical Records departments
  • Assists in office open/closing duties, office cleanliness, and unlocking/locking cabinets & doors and coverage for these responsibilities appropriately

 

Qualifications

  • High School Diploma required, some college preferred.
  • Demonstrated experience in customer service and clerical work.
  • Experienced in Microsoft Office products preferred
  • Donor software or EMR experience preferred
  • Previous healthcare experience preferred

As a Mid-Shift RN, you will experience a rewarding work life in a full time, Monday - Thursday position with the hours of 1:00pm -9:00pm.

Responsibilities

  • Working hours: Monday - Thursday 1:00pm - 9:00pm and weekend/holiday rotation as required for the role
  • Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI.
  • Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.
  • Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventions.
  • Working with community Physicians and the Hospice Medical Director to ensure that each patient has individualized medical care including medications, equipment, and supplies that work towards meeting patient and family goals.
  • Regularly administering care to a lower-than-average caseload.
  • Working creatively in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of medical care.
  • Educating patients, families, caregivers, and community members about hospice services, and issues related to end of life.
  • Providing exemplary care and service to all those you encounter in your work.

 

Qualifications

  • Current RN degree required; hospice, home care, case management or acute care experience required.
  • The successful candidate will be skilled in physical, emotional and spiritual symptom management while working in conjunction with an IDT and functioning independently in the field.
  • This position requires flexibility in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities.

As a Patient Access Specialist at Emmanuel Hospice, you will experience a rewarding work life while contributing to our team in a full time, hourly position.

Responsibilities

  • Processes all intake phone calls, electronic referrals, and walk-in requests for information about Emmanuel Hospice care and services.
  • Manages the referral process from initial communication to admission through coordination with patients, families, facility staff, doctors, and partners.
  • Gathers patient information from various sources, enters referral data into the electronic medical record, and develops hard chart copies.
  • Obtains physicians orders and medical records to support hospice services.
  • Verification of insurance and hospice benefit information.
  • Schedules appropriate staff to conduct informational/sign-on visits and nursing admission visits.
  • Builds relationships with referral sources as part of the Business Development team.
  • Coordination with complementary services team members (massage therapy, music therapy).
  • Assists with multi-line phone call management.
  • Communicates with patients’ interdisciplinary teams in an organized and timely manner.
  • Provides a calm, professional, and pleasant experience in person and over the phone for fellow staff, patients/families, facilities, vendors, and partners.
  • Performs other responsibilities as requested/assigned by the Director of Business Development

 

Qualifications

  • LPN license preferred
  • High School Diploma required, some college preferred
  • Previous healthcare experience required, hospice experience preferred
  • Experience in Microsoft Office products preferred
  • Positive, professional attitude with clear communication and customer service skills

  • Under the professional nurse’s direction, the Hospice Aide is responsible for providing personal hands-on care and activities of daily living.
  • Providing daily cares and comfort for the patient.
  • Working in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of spiritual, psychosocial, and physical care.

Qualifications

  • Certified as a Certified Nurse Aide in the State of Michigan
  • This position requires flexibility in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities.
  • Previous experience as a CNA required, Hospice or Home Care experience preferred.
  • The Hospice Aide should be able to respond sensitively to patient/family needs by observing and reporting any changes, including physical, spiritual, social and emotional, to a registered nurse or other members of the Interdisciplinary Team.
  • The Hospice Aide reports to the RN Case Manager and overall care is overseen by the Director of Clinical Services.

  • Assists with providing coverage and support for RN Case Managers, Extended Care Nurses, and Admissions Nurses during periods of increased need and to cover time off for full-time staff.
  • Providing Primary and On-Call Services triaging needs of patients and families through both phone contact and visits in the evenings and on the weekend.
  • Providing routine and crisis management for terminally ill patients and their families.
  • Conducting informational visits regarding hospice services.
  • Completing admission assessments as requested.
  • Communicating effectively with other hospice staff to ensure coordinated care is provided.
  • Shifts will vary based on the needs of the organization. Must be willing to work all of the varied shifts/roles in which care is provided by Emmanuel Hospice nursing staff.

Qualifications

  • The successful candidate will be a highly skilled and flexible clinician with excellent triage skills of individual’s physical, emotional and spiritual symptoms.
  • This position requires the ability to be a calming presence in crisis situations and effectively manage multiple calls at the same time.
  • This position requires flexibility in adapting to the changing needs of the patient and family.
  • Current RN degree and acute care experience required; hospice, home care, emergency room or ICU experience preferred.
