As Nurse Practitioner at Emmanuel Hospice, you will experience a rewarding work life while contributing to our team in a full-time (.8 FTE), salaried position.

Responsibilities:

  • Providing holistic care to the dying and their loved ones in a versatile 50-mile service area around Grand Rapids, MI.
  • Collaborating with and taking direction from the hospice physicians.
  • Completing recertification visits, face-to-face visits, and patient narratives.
  • Assisting with symptom management calls.
  • Completing timely and accurate documentation in the electronic medical record to coordinate care and determine hospice eligibility.
  • Communicating with referral sources, facility staff, patients and families to collaborate care.
  • Educating clinical staff on end-of-life issues and disease processes.

Qualifications:

  • Current Nurse Practitioner license in the state of Michigan required.
  • Possesses and maintains DEA Certification for prescribing controlled substances in accordance with delegation of authority from the collaborating physician.
  • Masters degree in Nursing with advanced skills in physical assessments.
  • Skilled in symptom management while working in conjunction with an interdisciplinary team and functioning independently in the field.
  • Flexibility in adapting to the changing needs of patients and families.
  • 5 years acute care experience preferred.
  • 2 years hospice or home care experience preferred.

  • Providing daily cares and comfort to the dying and their loved ones wherever they call home in territories within a 50-mile radius around Grand Rapids, MI.
  • The Hospice Aide is responsible for providing personal hands-on care and activities of daily living under the direction of a professional nurse.
  • Self-scheduling visits based on patient and family needs.
  • Working in unison with an Interdisciplinary Team, including the patient and their support system, to observe and report patients' spiritual, psychosocial, and physical needs.

Qualifications

  • Certified as a Certified Nurse Aide (CNA) in the State of Michigan
  • Must have a reliable vehicle to travel to patient visits.
  • This position requires flexibility in adapting to the changing needs of the patient and family with weekends (every 7th weekend), and holiday (1 per year) on-call responsibilities.
  • Previous experience as a CNA required, Hospice or Home Care experience preferred.
  • The Hospice Aide should be able to respond sensitively to patient/family needs by observing and reporting any changes to a registered nurse or other members of the Team.
  • The Hospice Aide reports to the RN Case Manager and overall care is overseen by the Director of Clinical Services.

As a Mid-Shift Hospice Aide at Emmanuel Hospice, you will serve alongside a dynamic team in a part time (hourly) position.

Responsibilities

  • Working a Monday - Friday 4:30-9:30pm schedule
  • Providing daily cares and comfort to the dying and their loved ones wherever they call home in territories within a 50-mile radius around Grand Rapids, MI.

As a Receptionist at Emmanuel Hospice, you will experience a rewarding work life in a full time, hourly position.

Responsibilities

  • General daily office management including; primary phone coverage, greeting & directing visitors/guests.
  • Assists the Director of Support Services with project management.
  • Assists staff/volunteers with clerical needs appropriately.
  • Ordering/maintaining an adequate amount of office supplies.
  • Communicates with other medical professional, patients/families, and facilities to coordinate the sharing of information in compliance with HIPAA.
  • Coordinates daily communication with the team.
  • Manages office open/closing duties for forwarding phones at 5pm, office cleanliness, and unlocking/locking cabinets & doors and coverage for these responsibilities appropriately.
  • Other responsibilities as requested by the Director of Support Services.

Qualifications

  • High School Diploma required, some college preferred.
  • Demonstrate experience in customer service, clerical work, and maintenance of medical records.
  • Experienced in Microsoft Office products with EMR experience preferred.
  • Previous healthcare experience preferred.

Responsibilities

  • Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI.
  • Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.
  • Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventions.
  • Working with community Physicians and the Hospice Medical Director to ensure that each patient has individualized medical care including medications, equipment, and supplies that work towards meeting patient and family goals.
  • Regularly administering care to a lower-than-average caseload.
  • Working creatively in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of medical care.
  • Educating patients, families, caregivers, and community members about hospice services, and issues related to end of life.
  • Providing exemplary care and service to all those you encounter in your work.

Qualifications

  • Current RN degree required; hospice, home care, case management or acute care experience required.
  • The successful candidate will be skilled in physical, emotional and spiritual symptom management while working in conjunction with an IDT and functioning independently in the field.
  • This position requires flexibility in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities.

As an Extended Care Weekend RN at Emmanuel Hospice, you will serve alongside a dynamic team in a full-time (salaried) position. Weekends-only shifts (9a-9p Friday-Sunday), plus remote availability on Monday evenings.

Responsibilities

  • Supporting needs of patients and families through both phone contact and visits in the evenings and on the weekends alongside triage nurses and other interdisciplinary support
  • Providing crisis management for terminally ill patients and their families
  • Conducting informational visits regarding hospice services
  • Completing admission assessments as requested
  • Communicating effectively with other hospice staff to ensure coordinated care is provided
  • Shift is every weekend (Friday - Sunday 9am-9pm)

Qualifications

  • Highly skilled clinician with excellent triage skills of individual’s physical, emotional and spiritual symptoms
  • Ability to be a calming presence in crisis situations and effectively manage multiple calls at the same time
  • Flexibility in adapting to the changing needs of the patient and family
  • Current RN degree and acute care experience required
  • Hospice, home care, emergency room or ICU experience preferred
