    As a Hospice Aide at Emmanuel Hospice, you will experience a rewarding work life while immersed in a non-profit ministry. Care and comfort for patients is viewed as an essential part of the experience for our patients and we are seeking Aides who can provide that extra measure of love and concern for their patients.

    Responsibilities and Duties

    As an Emmanuel Hospice Aide Your Responsibilities Would Include:

    • Providing holistic care to the dying and their loved ones in a versatile service area in a 50-mile radius around Grand Rapids, MI.
    • Under the professional nurse’s direction, the Hospice Aide is responsible for providing personal hands-on care personal care and activities of daily living.
    • Providing daily cares and comfort for the patient.
    • Working in unison with an Interdisciplinary Team, including the patient and their support system, in the planning, implementation, and evaluation of spiritual, psychosocial, and physical care.

    Qualifications and Skills:

    • Certified as a Certified Nurse Aide in the State of Michigan
    • This position requires flexibility in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities.
    • Previous experience as a CNA required, Hospice or Home Care experience preferred.
    • The Hospice Aide should be able to respond sensitively to patient/family needs by observing and reporting any changes, including physical, spiritual, social and emotional, to a registered nurse or other members of the Interdisciplinary Team.
    • The Hospice Aide reports to the RN Case Manager and overall care is overseen by the Director of Clinical Services.

    Your Responsibilities Would Include:

    • Supporting, coordinating, and overseeing organizational quality program. This includes tracking, trending, reporting, and coordinating with department leadership to ensure compliance with hospice conditions of participation, state regulations, CHAP standards and organizational policies and procedures.
    • Monitor and manage relevant benchmarking and quality data including but not limited to; PEPPER, HIS, Medicare Compare, and SHP
    • Monitors, reports on and plans for regulatory, legal and compliance changes impacting the mission of Emmanuel Hospice.
    • Planning and implementation, in conjunction with other team members, organizational Quality Assurance Initiatives.
    • Member of a dynamic Operational Leadership team that is responsible for day to day Operations.
    • Electronic Medical Record Super User
    • Manages the writing and revising of organizational policies and procedures.
    • Leading team and coordinating CHAP and other regulatory survey processes’.
    • Serving as organizational HIPAA/Privacy Officer.
    • Coordinating organizational Emergency/Disaster preparedness plan.

    Required Qualifications:

    The successful candidate will exemplify organizational and clinical while multi-tasking & maintaining focus in a fast-paced, rapidly growing environment. This candidate must also be proficient in Microsoft, Word, Excel, and electronic health records with the desire to learn and grow within their role. This position will work in conjunction with an IDT and management team to carry out the mission of Emmanuel Hospice. This position requires flexibility in adapting to the changing needs of the industry, organization, as well as our patients and families. A rotation in the manager on call schedule is required which includes some evenings, weekends and holidays. Must have a current license as a Registered Nurse with a minimum of 2 years nursing experience. Experience with Quality Assurance, Policy Development, or Compliance strongly preferred.

    Your Responsibilities Would Include:

    • Member of a dynamic Operations Leadership Team assisting with day to day leadership and clinical operations.
    • Supporting, coordinating and overseeing the day to day care of patients and families in a manner in keeping with the mission of Emmanuel Hospice and in compliance with hospice conditions of participation, state regulations, CHAP standards and organizational policies and procedures.

    • Developing a world-class Hospice Interdisciplinary Team in a manner consistent with the mission, vision and values of the organization.

    Required Qualifications:

    The successful candidate will exemplify organizational and clinical while multi-tasking & maintaining focus in a fast-paced, rapidly growing environment. This position will work in conjunction with an IDT and operational leadership team to carry out the mission of Emmanuel Hospice. This position requires flexibility in adapting to the changing needs of the industry, organization, as well as our patients and families. Current license as an RN or MSW in the State of Michigan or experience leading healthcare teams of greater than 30 people. Hospice experience or experience as a manager in a healthcare setting is preferred.

    Your Responsibilities Would Include:

    • Management and implementation of day-to-day fund development activities in order to raise funds for the operations and special programs of Emmanuel Hospice; including appeals, grants, events, donor cultivation and relationship building.
    • Manage and executes 2-3 fund development events logistics (1 major), including management of the event planning committee
    • Develop and implement strategies for major donor identification, cultivation, solicitation and stewardship. Plan and conduct personal face to face visits with major donors that include the Executive Director and/or Board of Directors when appropriate.
    • Oversee management of donor database
    • Assists with the facilitation of Emmanuel Hospice’s patient wish program by coordinating in-kind and financial donations.
    • Work collaboratively with the Executive Director and Committee to set and achieve fund development goals annually.

    Required Qualifications:

    The successful candidate will have a proven track record for implementation of a successful fund development program who can maintain focus in a fast-paced, rapidly growing environment. This candidate must also be proficient in Microsoft, Word, Excel, and have experience with donor management software. This position will work in conjunction with the Executive Director and board to ensure sustainability of the organization to carry out the mission. This position requires flexibility in adapting to the changing needs of the industry, organization, as well flexible hours related to the variable workload. Two years of fund development experience and a passion for interfaith hospice care required. Experience in healthcare preferred.

  • Hospice Aide – Mid-Shift

    Your RN Case Manager Responsibilities Would Include:

    • Providing holistic care to the dying and their loved ones in a versatile service area around Grand Rapids, MI.
    • Assessing the physical, psychological, social and spiritual needs of terminally-ill patients.
    • Assisting the patient and family with the Plan of Care to meet their needs and monitor goals, while suggesting appropriate, comprehensive and proactive interventions.

    Your RN Case Manager Required Qualifications: The successful candidate will be skilled in physical, emotional and spiritual symptom management while working in conjunction with an IDT and functioning independently in the field. This position requires flexibility in adapting to the changing needs of the patient and family with some nights, weekends, and holiday on-call responsibilities. Current RN degree required; hospice, home care, case management or acute care experience required.

    As a Per Diem Nurse at Emmanuel Hospice, you will experience a rewarding work life while immersed in a non-profit ministry with a full-time position. You will receive a competitive wage including mileage reimbursement.

    Your Responsibilities Would Include:

    • Assists with providing coverage and support for RN Case Managers, Extended Care Nurses, and Admissions Nurses during periods of increased need and to cover time off for full-time staff.
    • Providing Primary and On-Call Services triaging needs of patients and families through both phone contact and visits in the evenings and on the weekend.
    • Providing routine and crisis management for terminally ill patients and their families.
    • Conducting informational visits regarding hospice services.
    • Completing admission assessments as requested.
    • Communicating effectively with other hospice staff to ensure coordinated care is provided.
    • Shifts will vary based on the needs of the organization. Must be willing to work all of the varied shifts/roles in which care is provided by Emmanuel Hospice nursing staff.

    Required Qualifications

    The successful candidate will be a highly skilled and flexible clinician with excellent triage skills of an individual’s physical, emotional and spiritual symptoms. This position requires the ability to be a calming presence in crisis situations and effectively manage multiple calls at the same time. This position requires flexibility in adapting to the changing needs of the patient and family. Current RN degree and acute care experience required; hospice, home care, emergency room or ICU experience preferred.
